Thats where i am right now! Remember the bit wherw you go in the sewers and fight multiple spider guys. Instead of going in there, run past the sewer and take the left after.

About 37 hours in and loving it - i have realised that when i am summoned the bosses are harder and i get more souls for beating them! Never expected that!

Im up to the bonfire in Irithyll, its called Distant Manor, now I'm conscious that I havent seen or spoke to many NPC's even though there are loads.

Grey rat has just been sent out again.

Patches I met for the first time in rosaria's chamber, nowhere else, and he has since disappeared, not in the shrine, the guy is stuck down the well, the only ones I've really spoken to are the ones that appear in firelink shrine , I havent seen or heard from anyone.
Is there anything I can do to trigger any events or am i too far along in the story?

Still in the early game, can only play a few hours a week.

Wasted a good chunk of those few hours this week on the Abyss Watchers. First genuinely tedious Dark Souls boss I've encountered (though to be fair I didn't play much of 2). Can't be arsed with that. Got my souls, homeward bone'd out, will do something more interesting (I think I'm about to go into some kind of graveyard?) and come back when I'm overlevelled enough to just cheese through it. Hopefully there's enough stuff to do in the other part of the map that I won't have to come back here for a while...

Did you speak to Patches about Greirat after you sent him out thieving?
As for the well, if you bought the Catarina armour from Patches you can give it to him. If not, you MIGHT have missed his quest. If you kill another boss, other than the boss of the current area you might bring Patches back so you can get the armour.

I've only met patches in rosarias room. He isn't anywhere else. Unless he's somewhere else I'm not aware of. I've not been able to buy anything off him

Try going up the tower at Firelink walk to where you drop down to the area where you get the Fire Keeper clothes then walk back toward the exit. Patches should appear and lock you in. Die, drop down carefully or Homeward Bone out and he'll be upstairs at Firelink.

What do i do after the dancer? I have done both the irythll dungeon and anor londo.

You've collected the bowl, yes? Interact with the statue. (You should see game generated floor messages telling you that.)

Now the real fight begins, etc.

Before you fought dancer there was a old lady in the room with you. When she disappeared a bowl remained. You beat dancer and interact with the statue below the ladder. (The ladder which is opposite the entrance to the room.)
