If you have the HD pack then it will cost nothing extra per month. If you dont have it.. then I dont know sorry.

I imagine you'll have to subscribe to the HD Pack to receive it - there won't be an option to subscribe separately.
I wonder if you'll need the full Sky package to receive it, too. MTVN is available on the HD service despite not subscribing to the music pack, though that seems to be the only exception.
