You don't like it, you just opt out. I don't understand what people are getting upset about. You still have a choice.

I think there's a perception/concern/urban myth that doctors won't work so hard to save your life if you're an organ donor because they want you to due for your organs.

I don't understand it, but it's a reason I've seen a few times for people not wanting to be organ donors.

Given that doctors are evaluated on their performance and can even lose their career for letting a patient die from bad practice I fail to see much incentive. If doctor wants to progress, gain prestige and admiration then the best way to do it would be to have as close to a 100% survival rate as possible.
