I would guess that part of this comes about as part of just automated systems flagging things up and there is little to no human oversight/intervention (At least at any kind of leadership level that can actually make decisions) until things get to a very late stage.
It probably doesn't help that some people who are brought over to the UK as children and have lived here for decades make the assumption that they must be UK citizens/have leave to remain and nobody ever actually checks on their status. Simply being in the country for a long time doesn't give any automatic rights to becoming a citizen but there seems to be a perception that it does.
It certainly doesn't excuse what's going on mind, but I think there are reasons why cases like this are happening that aren't necessarily completely nefarious in nature, it's just that the system needs more human oversight and less reliance on automated flags.
