I think it's more than possible something is going on, either with her health or with their marriage but whatever it is, even if it is isn't anything substantial, it's hilarious how badly the Palace PR team are fumbling this.

I think it’s genuinely another person sitting in the chair. Zoom in to Kate’s neck and you can see the join line near the zip that then runs straight across.

That thing Louis is doing with his fingers is the sort of weird gooseberry fool every kid that age does with their fingers when they're bored. I just tried it now and can still do it (with added cracking noises obviously :dread:

I do think Kate’s face has been photoshopped into that photo. Look at the top of her head. There’s a V-shaped notch in her hair that looks like she’s been badly cut from another photo. And the lighting of her face doesn’t seem right.

The index finger looks a bit weird though, I'll give them that. And it looks like he's holding the arm of the chair with his thumb and little finger, which is unusual but not physically impossible. His shoulder hems look symmetrical to me.

Charlotte's different-looking heel is the tassle from her shoe zipper, but her wrist does look like it has been manipulated.

That's a good spot on the head-V, and yea the lighting does look a little weird. Her head looks like it's floating in some weird void.

The ridiculous thing about this photo is if something is wrong and they've had to mock up an image then why the hell have they bothered? Why did they need to release an image for mother's day? Yes I know there's a small number of weirdos out there that love these photos but the vast majority don't care and wouldn't even notice they haven't released one of their narcissist photos.
