Without looking it up, what's the name of the doctor played by Dan Castellaneta in the episode where Michael has appendicitis? The one who switches GOBs fingers around?

Then there's stuff like this:

And one that's new to me:

I've just started watching them again thanks to this thread, and noticed something in the pilot episode.
Gob scams George Michael out of $20, and George Michael winds up with a Monopoly board.
Later when Michael and George Michael are packing up their stuff in the display home's attic, George Michael has a pile of Monopoly boards.

Also, Buster's hand is foreshadowed loads, and whenever they mention something about hands there's a very subtle foreboding chime that's played. And sometimes you see like a huge splash of water in the background, if they're standing by the sea and mention hands.

Any time someone is sitting on a bench, there's something funny written on the bench. Can't remember the other one apart from Buster, but it's good.

Yes, which is why I didn't say first or second viewing, but fifth and sixth. If you've seen it more than four times and you're still picking up on stuff then you weren't paying attention the first few times.

I think maybe you're the one not watching closely enough

No, don't think so. The show's great without having to pretend that there's six or seven layers of subtlety hidden away.

Actually, I could probably forgive some of the Actor Allusions, like when Jessie calls George Michael 'Opie' and Ron Howard says something like 'Jessie had best watch her mouth'. If you didn't know that Ron Howard played Opie on The Andy Griffith Show, then you wouldn't get the reference. Not a particularly well-hidden reference though.

Then there's Jason Bateman's real life sister Justine Bateman playing his supposed sister in the episode Family Ties. Justine Bateman was in the show Family Ties in the 80s, and the fact that they're real life siblings is why Michael says something like 'yeah, that'd be weird on so many levels' when Nellie mentions them sleeping together. Though she's in the credits, so again, not really well hidden.

Have you not read any of this page?

Also, no-one's answer my question about the name of Dan Castenella's doctor character yet, proving either that 1) There is enough stuff hidden for a 6th or 7th watch or 2) No-one has watched the show 6 or 7 times.

I've no idea. Without looking it up I don't even know who Dan Castellaneta is.

I find that a lot of minor background actions can go completely unnoticed for several viewings. Maggie Lizer tapping the bowl of nuts instead of the evidence folder, then Michael subtly changing them around so the next time she taps the folder. Or Gob throwing the broom into the closet, the broom hitting the light bulb and breaking it. Those two I didn't notice the first 5 or 6 times.

And taf, that monopoly bit is amazing  . Another thing I didn't notice in something like 6 rewatches. Unfortunately I do remember the full name of Dr. Stein

That's probably my favourite moment in AD.

Blatantly the best scene. Fantastic set-up throughout the episode, with the payoff to match it.
