The way I see it, the British Army doesn't have lootenants and the American Army doesn't have leftenants, so it'd be a bit rude to get that deliberately wrong if you were speaking to someone holding that rank. For fictional characters go with what they're called in canon (e.g. If you say leftenant Worf you sound like a prick. For a phrase like "trusted lieutenant" I would say "lef."

Normally the context makes the pronunciation obvious.

The word in a vacuum I would pronounce "loo" (although you wouldn't be able to hear me)
