I presume the only reason they don’t fake it these days is because it’s much cheaper to fire a real gun? With all the computer graphics available at their disposal, making a gun fire when it needs to be pointed at someone would be easy.

It will be negligence from whomever prepared the gun. Bruce Lee's son died in a similar way.
In the scene preceding the fatal scene, the gun was loaded with improperly-made dummy rounds, cartridges from which the special-effects crew had removed the powder charges, so in close-ups the revolver would show normal-looking bullets. However, the crew neglected to remove the primers from the cartridges. At some point before the fatal event, one of the rounds had been fired; although there was no powder charge, the energy from the fired primer was enough to push the bullet into the gun barrel, where it got stuck (a condition known as a squib load). For the fatal scene, which called for the revolver to be fired at Lee from a distance of 3.6–4.5 meters (12–15 ft), the dummy cartridges were replaced with blank rounds, which contained a powder charge and primer, but no bullet, allowing the gun to be fired without the risk of an actual projectile. When the blank round was fired, the bullet lodged in the barrel was propelled forward with almost the same force as if the round were live, and it struck Lee in the abdomen.
I was going to bring up the Brandon Lee thing. I'm surprised they don't adopt the approach used in the force awakens, where the storm trooper guns had gas in them to give recoil, giving the impression something was being fired. Just putting CG gunfire on to take guns looks terrible (see the walking dead).
That’s a shame, always seemed like a really classy guy and helped Scotland to turn a corner, albeit briefly, when in charge. Remember feeling sorry for him when Rangers didn’t win 10 in a row and their fans walked out of the ground early even though it was his last match in charge.
He was manager of Everton during a pretty difficult time finance/boardroom wise and while the football played was, understandably, dour at times he always acted with class and dignity. Was instrumental in getting Peter Johnson out after he sold Ferguson behind his back.
